An accident is the
wrong time to find out.

Health and safety duties don't fail quietly — they fail at the worst possible moment, in front of an inspector, an insurer or a tribunal. The HSE can prosecute, and "we meant to" is not a defence. The difference between a manageable incident and a serious liability is almost always whether you can produce the record: the assessment, the controls, the report, the training.

No risk assessment on file
A suitable and sufficient risk assessment is a legal duty. If it isn't written down, scored and reviewed, you can't show you ever met it.
A missed RIDDOR report
Certain injuries and dangerous occurrences must be reported to the HSE within strict deadlines — and missing one is an offence in its own right.
